We know your health needs may go beyond medical exams and check-ups. Our trained and caring staff can help you make sense of healthcare paperwork and assist with applications for health insurance, and food stamps, as well as connect you with resources to help you live well and do what you love.
Health insurance enrollment assistance is available to all members of the community. The service is free and confidential
Here’s what you should bring:
• An email address where you can receive messages about your HealthCare.gov account
• Your mailing address
• Birthdates for you and members of your household whom you’d like to enroll
• Social Security numbers -or- Alien Resident numbers (for those who have one)
• Your gross household income (from pay stubs or W-2 forms)
• Information about health insurance available to you through an employer
You will not be asked about pre-existing medical conditions.
