1st Annual Cycle for Community Health

Coast Community Health Center invites you to join our 1st annual Cycle for Community Health, a fundraiser to support the expansion of wellness services to southern coast residents.

Coast Community Health Center’s ride will treat your sights to the beautiful coast, and your excite your skill with pleasant hills and turns.

Cyclists’ proceeds from this ride will go directly to support the expansion of vital health services to Southern Oregon Coast residents as well as visitors.

Year-Round

Coast Community Health Center enrollment assisters and outreach workers are trained to help families and individuals check their eligibility and enroll or re-enroll in OHP or Federal Marketplace plans online at HealthCare.gov.

Want to Speed Your Insurance Application?

Here’s what you should bring.

• An email address where you can receive messages about your HealthCare.gov account

• Your mailing address

• Birthdates for you and members of your household whom you’d like to enroll

• Social Security numbers -or- Alien Resident numbers (for those who have one)

• Your gross household income (from pay stubs or W-2 forms)

• Information about health insurance available to you through an employer

You will not be asked about pre-existing medical conditions.

Our outreach staff are available each week at Bandon and Port Orford/Langlois public schools.

Students and their parents or guardians can request an appointment or just drop by.

We can share information about health care and programs to help students stay healthy. Ask us if children or adults in your home qualify for free or reduced cost services.

Transportation Assistance & Personal Care

• or set up a pick up or drop off.

More Transportation Options

Patients not enrolled with OHP may qualify for gas cards or bus passes. Dollar amounts are based on your driving distance. To get started, we’ll complete a Patient Transportation Assistance form.

Personal Care

Let us know if you need help with personal care.

We can help with showers and laundry. We can also assist with emergency goods, such as food, toiletries, blankets and clothing.

Showers and laundry assistance are by appointment. Talk with your provider, or contact outreach staff, to plan a visit.
