Meet your wellness goals

Let our outreach staff help you find health resources and human services for adults and children– at our health center and in the community. Visit us at our Bandon clinic or Port Orford office.

We know your health needs may go beyond medical exams and check-ups. Our trained and caring outreach staff can

help you make sense of health care paperwork
assist with applications for health insurance, food stamps, and housing, and
connect you with resources to help you live well and do what you love.

How are we doing?

If you have used our enrollment services with the Federal Marketplace or Medicaid (OHP), please complete our short 6 question survey. We appreciate hearing from you!
