Coast Community Health Center outreach and enrollment specialists are trained to help families and individuals check their eligibility and enroll or re-enroll in OHP or Federal Marketplace plans online at HealthCare.gov.

Want to Speed Your Insurance Application?

Here’s what you should bring.

• An email address where you can receive messages about your HealthCare.gov account

• Your mailing address

• Birthdates for you and members of your household whom you’d like to enroll

• Social Security numbers -or- Alien Resident numbers (for those who have one)

• Your gross household income (from pay stubs or W-2 forms)

• Information about health insurance available to you through an employer

You will not be asked about pre-existing medical conditions.
