If you’d like to join our practice, feel free to pick up patient packets at the health center, or use our Pre-Register form. The Pre-Register button is located at the bottom right of the ECW Patient Portal log-in page. You may expect a follow-up phone call within 2 business days.

Patient services staff can offer assistance, before your appointment, if you would like help completing required forms.

Your privacy is important to us. Health center staff will not communicate or respond to health care related messages using email or other non-encrypted electronic communication.

Payment and Insurance

A minimum copay is requested at the time of service. We accept cash, checks, major credit and debit cards.

Office visit copay amounts are set by your insurance company.

Private pay is welcome. Ask about our sliding scale program when you call for your appointment. Download your sliding scale application.

Health Services Referrals

Coast Community Health Center is your first stop for health check-ups and wellness advice. Regular visits with your primary care doctor or nurse practitioner can help you stay healthy.

When you need the attention of a specialist, we’ll help with referrals to qualified local and regional medical and allied health professionals.

To request a referral, speak with your doctor or nurse practitioner. Patients are responsible for authorizing access to medical records and payment for services by the referred professional.

Prescriptions

Coast Community Health Center Pharmacy

As a federally funded community health center, our 340B pharmacy provides prescriptions at a reduced cost by passing rebates from pharmaceutical manufacturers to our patients. Patients receive the quality services of a tenured pharmacy director while saving money on prescriptions.

To start an account, ask your health center provider to send your next prescription to our pharmacy. To transfer existing prescriptions, contact Coast Community Health Center pharmacy at 541-347-2724.

To fill prescriptions from another provider or specialist, tell your specialist that you have changed to the health center pharmacy. New prescriptions from a specialist can be filled at our pharmacy with approval from a Coast Community Health Center provider.

At this time, our pharmacy can only fill prescriptions for Coast Community Health Center patients. The service is a benefit of the Health Resources Services Administration of Federally Qualified Health Center programs.

Filling and Paying for Prescriptions

Coast Community Health Center has a two business day refill policy. Please plan ahead for prescription refills, and ask your pharmacy to send a refill request to the clinic.

Our providers may require an office visit or lab services before prescribing refills for some medications.

Coast Community Health Center may not be able to provide Prior Authorizations for all medications.

If a prescription is not covered by your insurance plan formulary, contact your insurance company for information about comparable alternative prescriptions. Find the customer service telephone number for your insurance company on the back of your insurance card.

OHP patients can download the Oregon Health Plan list of approved prescriptions.

If you need help paying for your medication, ask our staff about prescription assistance programs. Medicare patients may contact SHIBA for prescription information.
